About AKHS-A (Aga Khan Health Service-Afghanistan):

Aga Khan Health Services (AKHS) is one of the most comprehensive private, not‐for‐profit health care systems in the developing world with community health programs in large geographical areas in Central and South Asia, as well as in East Africa.

The Aga Khan Health Services, Afghanistan (AKHS, A) was established in 2003 as part of the Aga Khan Development Network (AKDN). AKHS, A provides health care services mainly on behalf of the government including the Ministry of Public Health’s of Afghanistan’s Basic Package of Health Services (BPHS) and Essential Package of Hospital Services (EPHS). AKHS, A is unique both in scale and scope, delivering critical health services along the continuum of care, from Community Health Workers (CHWs) to ISO certified hospital facilities.

AKHS, A is currently implementing healthcare services in Bamyan, Baghlan, Badakhshan, Kabul and Takhar provinces. The agency’s scope of work includes the provision of primary and secondary healthcare, preventive and curative medical care, Nursing and Midwifery education, Human Resources Development and Hospital Management. As BPHS and EPHS implementer, AKHS, A provides technical support as well as capacity building of health personnel, immunization services, nutrition trainings and the training of Community Health Nurses and Community Health Midwives.

AKHS, A draws on internal and other donor fundings to continue supplementary and complementary support to the national health system. Moreover, AKHS, A has introduced innovations, including, expansions of e-Health services, e-learning initiatives, and cross border arrangements. To promote healthy environment, AKHS, A is also implementing net zero and greenhouse gas emission initiatives.

اداره عمومى کلینیک

کنترول از حاضری پرسونل مربوط مرکز صحی و پیشبرد امورات اداری آن

مسول تشکیل کمیته راپور دهی و تنظیم و راپور دهی ازاداره سیستم معلومات صحی (HMIS)

آگاه بودن از رهنمودها و پروتوكول های جدید ملى

مسول اداره وتنظیم مجالس داخلی مرکزصحی

نظارت از فعالیت ها در حفظ مراقبت تعمیر و نظافت کلینیک

مسؤل اداره ونظارت از تطبیق ستندرد های وقایه ازانتان در بخش های مربوطه کلینیک

تطبیق درست پروگرام IMCI و نظارت ازبخش ORT corner

ارایه خدمات با کیفیت امراض چشم برای مریضان مربوطه

خدمات نقل الدم برای مریضان نیازمند

تطبیق و رعایت دقیق ستندرد های وقایه از انتان .

تهیه و تعقیب پلان برای فعالیت های دورس (Outreach) و معافیت کتلوی

نظارت از فعالیت های کارکنان صحی جامعه و پوسته های صحی حداقل ماه یک مرتبه به کمکCHS مربوطه.

نظارت از فعالیت های CHS مربوطه مطابق به لایحه وظایف وپلان داده شده

اشتراک در شورای صحی به سطح کلینیک

مسول تطبیق پروگرام بهبود کیفیت و تعقیب آن باهماهنگی جامعه

واقعه یابی، جمع آوری نمونه بلغم مریضان مشکوک ،راجستر,تعقیب و تداوی مریضان توبرکلوزمطابق به رژیم DOTs و راپور دهی ماهانه و ربع وار مریضان توبرکلوز

اداره و راپوردهی منظم و بموقع از ادویه ضد توبرکلوز

تنظیم توزیع مواد غذائی کمکی برای مریضان توبرکلوز و راپور دهی منظم و به وقت آن به دفتر

تعلیم وآگاهی دادن به CHW در مورد واقعات توبرکلوز تحت تداوی که به کلنیک دسترسی ندارند و تعقیب منظم آن توسط CHW

تداوی مریضان ملاریا مطابق به ستندرد ملی کنترول ملاریا

بهبود بخشیدن آگاهی عامه درمورد HIV/ AIDS

تبلیغات صحی، واقعه یابی، مشوره دهی، تداوی، راجستریشن, راپور دهی منظم، تعقیب و راجع ساختن مریضان روانی و معلولین مطابق به رهنمود BPHS به مراکز صحی بالاتر

مسول بودن فعال کمیټه تایید شده برای تحلیل فعالیت های برانامه P4P مشارکت نماید

بر اساس فعالیت هایی مرکز صحی برای دست اورد های ماهانه شاخص های P4P سهم فعال داشته باشد

به عنوان مسول مدیریت شاخص های مربوط P4P و غیر ان در مرکز صحی سهم فعال داشته باشد

مسول ارایه خدمات صحی باکیفیت ،راجستریشن دقیق ، جمع اوری ارقام ،تهیه راپور ها (دقیق و به موقع ) فعالیت داشته باشد

مسول اریه خدمات صحی مطابق به رهنمود ‍BPHS (شناسایی واقعه ، مشوره دهی ،تداوی و تعقیب )با ثبت کامل مشخصات مریضان /مراجعین (نام ، ادرس مکمل ،شماره تماس )میباشید ،برای تاید هویت مراجعه کننده.

شاخص هایی QQMو QQC را نظارت نموده و به طور فعال به کارمندان کومک نماید <

شاخص هایی غیر P4P را به کمک کارمندان مربوطه به طور جدی تعقیب نماید

اشتراک در طرح وتطبیق بروگرام هاى آموزشى جريان كار برای پرسونل کلینیک

اشتراک در فعا لیت های کمپاین واکسینیشن

راپوردهی فعال و به موقع امراض فوراَ قابل گزارش (AFP،, مرس سگ دیوانه , کولرا AEFI & Neonatal Tetanus , Measles)

سهم گیری فعال درتطبیق ستندرد های جدید بهبود کیفیت
