Job Purpose
The Case Management and MHPSS Officer is responsible for the identification, intake, assessment, planning, management, follow-up, referral, and closure of cases among Marastoon beneficiaries. In accordance with the Marastoon Case Management Guidelines, the position assesses the individual needs of beneficiaries, develops and implements individualized case plans, follows up on service provision and referrals, and ensures that case and beneficiary information is accurately, systematically, timely, and confidentially recorded and managed.
Duties and Responsibilities
- Implement the Marastoon Case Management Guidelines and carry out all stages of Case Management, from identification and intake through assessment, planning, service provision, follow-up, and case closure.
- Identify and register beneficiaries requiring Case Management services and conduct initial and comprehensive assessments to identify their needs, concerns, capacities, vulnerabilities, and risk factors.
- Develop an individualized case plan for each beneficiary based on their identified needs and circumstances, in consultation with the beneficiary, including agreed objectives, actions, and required services.
- Manage and regularly follow up on assigned cases, assess progress against individualized case plans, monitor changes in beneficiary circumstances, identify challenges, and determine necessary follow-up actions.
- Identify complex, sensitive, and high-risk cases and refer them to the Case Management and Psychosocial Counselling Specialist for appropriate technical guidance and professional support.
- Refer beneficiaries to appropriate health, psychosocial, legal, protection, livelihood, education, and other relevant services, and follow up on referrals to ensure that required services are received.
- Coordinate with psychosocial counsellors, social workers, health, legal, livelihood, education, and other relevant departments to facilitate appropriate and coordinated services for beneficiaries.
- Accurately, systematically, and timely record beneficiary and case information, including assessments, individualized case plans, actions taken, referrals, follow-up activities, and outcomes in the Marastoon beneficiary information and Case Management system.
- Maintain strict confidentiality and information security and ensure that personal and sensitive beneficiary information is accessed, used, and shared only when necessary and in accordance with authorized procedures and data protection principles.
- Conduct regular meetings and follow-up sessions with beneficiaries to review their situation, obtain feedback, identify changes, and update individualized case plans and case information as required.
- Prepare regular reports on case status, including active and closed cases, services provided, referrals, challenges, and beneficiary needs, and submit them to the relevant supervisor.
- Support the collection and analysis of case information to identify common challenges and needs among beneficiaries and provide practical recommendations for improving Marastoon services.
- Support the implementation of the beneficiary Exit Strategy and ensure that case closure takes place when agreed objectives have been achieved, needs have been adequately addressed, and the beneficiary is appropriately prepared for independent living and social reintegration.
- Apply the principles of do-no-harm, safeguarding, informed consent, human dignity, and prevention of abuse, exploitation, and harassment throughout all interactions with beneficiaries, and report protection concerns through established mechanisms.
- Participate in case review meetings, training and capacity-building activities, support the Case Management team, and perform other duties relevant to the position as assigned by the supervisor.
- Give due and practical attention to all religious and cultural requirements, including appropriate observance of Hijab in the workplace and during engagement with beneficiaries.
- Ensure the proper and responsible use, care, and protection of all equipment, materials, supplies, and vehicles assigned to the relevant department.
- Perform other duties related to the position as assigned by the supervisor or head of the relevant department in accordance with the applicable laws of the Afghan Red Crescent Society and the procedures and policies of the Human Resources Department.
